Be it Remembered That on this day to-wit the Fifth of February 
in the year of our Lord One Thousand Eight Hundred and Ninety 
Seven One Hundred and 21 of the independence of the United 
States of America, before me, S. R. Olliphant, MD, President 
Board of Health and Ex-Officio Recorder of Births, Deaths, 
Marriages, in and for the Parish and City of New Orleans, 

personally appeared 
ROSARIO CARUBBA, native of Province of Palermo, Italy residing 
at 918 Teche Street in Algiers in this city, who hereby declares 
that on the First day of December last year (1 December 1896) at 
his residence was born a female child, named 

			PAOLA CARUBBA (white) 

Lawful issue of Deponent a native of Italy aged 26 years, occupation 
Shoemaker and Carmela Palisi (sp Pallise incorrectly) a native of 
Province of Palermo, Italy, aged 20 years, 

Thus done at New Orleans, in the presence of the aforesaid R. CARUBBA 
as also in that of Messrs. P. H. Lanauze and Jas. T O Dowd both of this 
city witnesses, by me requested so to be, who have hereunto set their 
hands together with me, after due reading hereof, the day, month and 
year first above written.
