Be it remembered that on this day, to wit, the Tenth of January 
in the year of our Lord One Thousand Eight Hundred and Ninety 
four and the one hundred and Eighteenth of the independence of 
the United States of America, before me, S. R. Olliphant, MD, 
president Board of Health and Ex-Officio Records of Births, Deaths, 
and marriages in and for the Parish and City of New Orleans personally 
appeared 

John E. Coyne, a native of LA residing Algiers, LA who hereby declares 
that on the Twentieth of December 1893 (Dec 20/93) at his residence was 
born a male child, named 

		Philip Mathew Coyne (White) 

lawful issue of Deponent a native of LA aged 31 years, occupation Engineer 
and 
Julia Mary Nolan, a native of LA aged 29 years.  

This done at New Orleans in the presence of the aforesaid John E. Coyne 
as also in that of Messrs. Ph. H. Lanauze and M. Gu?rand both of this 
city witnesses by me requested as to be who have hereunto set their hands 
together with me after reading hereof, this day, month and year first above 
written.
