Be it Remembered, That on this day to wit: the  tenth of October 
in the year of our Lord, One Thousand, eight Hundred and eighty 
eight and the One Hundred and thirteenth of the Independence of 
the Untied States of  America, before me, C Wilkerson  M.D., 
Chairman of the Board of Health and Ex-Officio Recorder of Births, 
Deaths and Marriages, in and for the Parish of Orleans, and the city 
of New Orleans, 

Personally appeared 
O L Heinsz native of this city residing at Jersey near Milan Streets 
who Hereby declares that on the first instant (10-01-1888) was born 
a female child named 

			Fullmer ( white ) 

issue of Charles E Fullmer a native of this city age 23 years, 
occupation laborer and Louisa Heinsz a native of this city age 
23 years.

Thus done at New Orleans, in the preasence of the aforesaid O L 
Heinsz as also in that of P H Lanauze & ????? both of this city, 
witnesses by me requested so to be, who have set their hands together 
with me, after reading hereof this day, month and year first above written.
