be it remembered that on the day to wit the twenth sixth day of 
August in the year of our Lord One Thousand eight hundred and 
ninety one and the one hundred and sixtenth of the independce 
of the Untied states of america before me S R Olliphant MD 
president Board of Health and ex-officio of Births, Deaths and 
marriages in and for the parish and city of New Orleans 

personnally appeared
Charles E Fullmer native of this city residing cornor of 
Constantinople & Tchoupitoulas who hereby declares that 
on the ninetenth day of this present month (08-19-1891) 
at his residence was born a male child names

				Charles E Fullmer Jr (white)

lawful issue of deponent a native of this city aged 26 years 
occupation Laberor and Louise E Heintz a native of this city 
aged 26 years.

Thus done at New Orleans in the presence of the aforesaid C E 
Fullmer as also that of Messrs: P H Lanauze and A A Vonhiss???? 
both of this city witnesses by me requested so to be who have 
here unto set their hands together with me after due reading 
here of the day, month abd year first above written.
