Be it remembered that on this day to wit, the twenty-first of January in the
year of our Lord one thousand eight hundred and ninety-six and the one
hundred and 20 of the Independence of the United States of America, before
me, S. R. Olliphant, M. D., President Board of Health and Ex-Officio
Recorder of Births Deaths and Marriages in and for the Parish and City of
New Orleans personally appeared

Benjamin Fontenette a native of La. residing #1613 Dumaine St. who hereby
declares that on the sixth of June 1893 (June 6th/93) at his residence was
born a female child, named

			Lucy Fontenette

lawful issue of Deponent a native of La. aged 24 years, occupation
Restaurant and Octavie Legier a native of this city, aged 25 years,

Thus done at New Orleans in the presence of the aforesaid B. Fontenette as
also in that of Messrs. P. H. Lanauze & L. B. ???? both of this city,
witnesses by me requested so to be, who have hereunto set their hands
together with me, after due reading hereof the day, month and year first
above written.
