Be it Remembered, That on this day to wit: the  twenty first of 
Februaryin the year of our Lord, One Thousand, eight Hundred and 
Seventy six and the One Hundredth of the Independence of the 
Untied States of America, before me, G Casanave M.D., Chairman 
of the Board of Health and Ex-Officio Recorder of Births, Deaths 
and Marriages, in and for the Parish of Orleans, and the city of 
New Orleans, 

Personally appeared 
Louisa Heinsz born Fabian native of Germany residing in this city 
who Hereby declares that on fourth day of February eighteen hundred 
seventy six ( 02-04-1876 ) at half past nine am was born in this city 
a male child named 

			Edward John Heinsz 

lawful issue of deponent with Otto Edmond Heinsz a native of Prussia

Thus done at New Orleans, in the preasence of the aforesaid Louisa 
Heinsz as also in that of Pager & Colin both of this city, witnesses 
by me requested so to be, who have set their hands together with me, 
after reading hereof this day, month and year first above written.
