Be it Remembered, That on this day to wit: the  16th of March
in the year of our Lord, One Thousand, eight Hundred and ninty 
three and the One Hundred and  sevententh of the Independence 
of the Untied States of America, before me, S R Olliphant M.D.,
Preseident of the Board of Health and Ex-Officio Recorder of 
Births, Deaths and Marriages, in and for the Parish and the city 
of New Orleans, Personally appeared

Margaret Waschmann native of this city residing at  #577 Natero St
who Hereby declares that on the thirtenth of January 1893 (01-13-1893)
was born a male child named

			FREDERICK WILLIAM HEINSZ

(white) issue of Otto Heinsz a native of New Orleans ahe 23 years 
occupation laborer and Sophie Rebb a native of New Orleans age 22
years. 

Thus done at New Orleans, in the preasence of the aforesaid _____ 
as also in that of ________ both of this city, witnesses by me 
requested so to be, who have set their hands together with me, 
after reading hereof this day, month and year first above written.
