Be it Remembered, That on this day to wit: the tenth of May 
in the year of our Lord, One Thousand, eight Hundred and 
ninety seven and the One Hundred and twenty one of the 
Independence of the Untied States of America, before me, 
S R Olliphant M.D., Chairman of the Board of Health and 
Ex-Officio Recorder of Births, Deaths and Marriages, in and 
for the Parish of Orleans, and the city of New Orleans, 

Personally appeared 
Geo A Heinsz native of Louisiana residing at # 4015 Tchoupitoulas
who Hereby declares that on fourth day of August 1895 (08-04-1895)
at his residence was born a male child named 

			George Richard Heinsz

issue of deponent a native of Louisiana age 26 years, occupation 
Carpentar and Josephine Bougard a native of Louisiana age 21 years.

Thus done at New Orleans, in the preasence of the aforesaid G A 
Heinsz as also in that of P H Lanauze & R Paul both of this city, 
witnesses by me requested so to be, who have set their hands together 
with me, after reading hereof this day, month and year first above written.
