Be it Remembered, That on this day to wit: the twenty seventh of 
May in the year of our Lord One Thousand eight Hundred and ninety 
seven and the One Hundred and 21 of the Independence of the Untied 
States of  America, before me, S R Olliphant M.D., President of the 
Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages, 
in and for the Parish of Orleans, and the city of New Orleans, 

Personally appeared 
Theodore J P Heintz native of La residing at No 3704 Tchouapitoulas 
Street who hereby declares that on the Fourth day of January 1897 
(4 Jan 1897) at his residence a female child named 

		Louisa Heintz (White)

lawful issue of deponent a native of La aged 31 years occupation Driver 
and Mary Bitter a native La aged 25 years.

Thus done at New Orleans, in the preasence of the aforesaid  T H P Heintz 
as also in that of  P H Lanauze & R Paul both of this city, witnesses by 
me requested so to be, who have set their hands together with me, after 
reading hereof this day, month and year first above written.
