Be it Rememberd that on the day to wit the Fourtenth of November 
in the year of our Lord One Thousand Eight Hundred and ninety three 
and the One Hundredand eightenth of the independence of the Untied 
States of America before me S R Olliphant M D President Board of 
Health and Ex-Office of births, Deaths and Marriages in and for 
the Parish and city of New Orleans

personally appeared
Felicien Fullmer Lozes native of this city residing in this city who 
hereby declares that on the tenth day of May 1893 (10 May 1893) at 
his residence was born a male child named

			Frederick Louis Lozes (White)

lawful issue of deponet a native of this city aged 23 years occupation 
butcher and Anna A Willmes a native of this city aged 23 years.

thus done in New Orleans in the presence of the aforesaid F F Lozes as 
also in that of P H Lanauze & ????? both of this city, witnesses by me 
requested so to be, who have set their hands together with me, after 
reading hereof this day, month and year first above written.
