Be it Remembered, That on this day, to-wit:  the Sixth of October in
the year of our Lord 1893 and the One Hundred and 11th of the Independence
of the United States of America, before S.R. Olliphant, M. D.,
Chairman Board of Health and Ex-Officio Recorder of Births,
Deaths and Marriages, in and for the Parish and City of New Orleans,
personally appeared:

C. J. Miltimore, native of this city residing at No. 389 C????? street who
hereby declares, that on the 29th day of April 1893 (APril 29, 1893) at her
residence was born a Female child, named

		Fanny Miltimore (white)

lawful issue of Theodore J Miltimore, a native of this city aged 35 years,
occupation Clerk and Elizabeth Doyle a native of this city, aged 27 years.

Thus done at New Orleans, in the presence of the aforesaid C. J. Miltimore
as also in that of Messes ??????????? both of this City,
witnesses, by me requested so to be, who have here unto set their hands
together with me, after due reading hereof, the day, month and year
first above written.
