Be it Remembered, that on this day to wit: the Sixteenth of 
January in the year of our Lord One Thousand, Nine Hundred 
and fourteen and the one hundred and 38th of the independence 
of the United States of America, before me, Wm. T. O'Reilly, 
M.D. Chairman Board of Health and Ex-officio Recorder of Births, 
Deaths, and Marriages, in and for the Parish and City of New Orleans, 

personally appeared 
Mrs. I. Russell, a native of Louisiana residing at no. 931 Evelina 
street who hereby declares, that on the third of this month (3 Jan 
1914) at her residence was born a male child, named 

			Ernest Peterson Spellman (White) 

lawful issue of John P. Spellman a native of Louisiana aged 41 years, 
occupation Machinist and Coralee (Error: Should be Carrie Peterson) 
Petterson (sic) a native of Louisiana, aged 42 years. 

Thus done at New Orleans, in the presence of the aforesaid Mrs. I. 
Russell as also in that of PH. H. Lanauze & J. Nedes (?) both of 
this city witnesses by me requested so to be, who have herewith 
set their hands together with me, after due reading hereof, the day, 
month, and year first above written.
