T. O'Reilly, M.D. Chairman Board of Health and Ex-officio Recorder of Births, Deaths, and Marriages, in and for the Parish and City of New Orleans, personally appeared Mrs. I. Russell, a native of Louisiana residing at no. 931 Evelina street who hereby declares, that on the third of this month (3 Jan 1914) at her residence was born a male child, named Ernest Peterson Spellman (White) lawful issue of John P. Spellman a native of Louisiana aged 41 years, occupation Machinist and Coralee (Error: Should be Carrie Peterson) Petterson (sic) a native of Louisiana, aged 42 years. Thus done at New Orleans, in the presence of the aforesaid Mrs. I. Russell as also in that of PH. H. Lanauze & J.
