Be it Remembered, that on this day, to-wit: the Twenty seventh of 
February in the year of our Lord One Thousand Eight Hundred and 
Ninety nine and the one Hundredth and 23 of the Independence of 
the United States of American, before me, Kohnke M.D., Chairman 
Board of Health and ex-officio Recorder of Births, Marriages, and 
Deaths, in and for the Parish and City of New Orleans, 

personally appeared
William Rudolph Smith native of this city residing at No 2927 Royal 
Street who hereby declares that on the ninetenth day of February 1899 
(19 Feb 1899) at his residence was born a male child named 

			Milton Frederick Joseph Smith

Thus done at New Orleans in presence of the aforesaid W R Smith as 
also in that of Messrs P H Lananza & Jos T O'Dowd both of this city 
witnesses by me requested so to be who have hace hereafter set their 
hands together with me after due reading hereof, the day month and 
year first above written.
