Be it Remembered, That on this day to wit: the Elventh of Ocotber in 
the year of our Lord One Thousand eight Hundred and eighty four and 
the One Hundred and Sixtenth of the Independence of the Untied States
of America, before me, Joseph Holt M.D., President of the Board of 
Health and Ex-Officio Recorder of Births, Deaths and Marriages, in 
and for the Parish of Orleans, and the city of New Orleans, 

Personally appeared 
Zacharie Surbeck native of Switzerland residing at Royal and Mazant 
St who hereby declares that on the twenty third day of last month 
(Sept 23 1884) at his residence a male child named 

		Jean Jacques Surbeck (White)

lawful issue of deponent a native of Switzerland aged 57 years 
occupation Carpenter and Francisca Zoller a native Germany aged 
30 years.

Thus done at New Orleans, in the preasence of the aforesaid Z Surbeck 
as also in that of P H Lanauze & B B Howard both of this city, 
witnesses by me requested so to be, who have set their hands together 
with me, after reading hereof this day, month and year first above written.
