of June in the year of our Lord One Thousand Nine Hundred and 
Four and the One Hundred and 28 of the independence of the United 
States of America, before me, Quitman Kohnke, M.D., Chairman Board 
of Health and Ex-Officio Recorder of Births, Deaths and Marriages 
in and for the Parish and City of New Orleans,

Salvatore Tedesco, native of Palermo, Italy residing at No. 327 N. 
Galvez Street who hereby declares that on the thirty first of last 
month,31 May 1904 at his residence was born a male child named

occupation Sailor and Rosa Cristina a native of this city, aged 27 years
