Be it Remembered, That on this day to=wit the 2 of October in the year of
our Lord One Thousand Nine Hundred and Twenty fTwo and the One Hundred and
47 of the independence of the United States of America before me John
Callan, M.D. President Board of Health and Ex-Officio Recorder of Births,
Deaths and Marriages, in and for the Parish and City of New Orleans,
personally appeared :

D. A. Ingersoll an undertaker, a native of this city residing at 442 Jackson
Ave. who hereby declares, that

a native of Tolland C. Connecticut . aged 83 years,
departed this life, ??????? (1 Oct 1922) at 929 First St. in this city

Deceased was the husband of Charlotte Doyle. 50 ??????

Thus done at New Orleans, in the presence of the aforesaid D. A. Ingersoll
as also that of Messes P H Lanauze & W. ? Prudhomme both of this city
witnesses, by me requested so to be, who have hereunto set their hands,
together with me, after due reading hereof, the day, month and year first
above written.
