Be it Remembered, That on this day to=wit the 9 of December in the year of
our Lord One Thousand Nine Hundred and Twenty five and the One Hundred and
50 of the independence of the United States of America before me W. H.
Rodin, M.D. President Board of Health and Ex-Officio Recorder of Births,
Deaths and Marriages, in and for the Parish and City of New Orleans,
personally appeared :

D. A. Ingersoll an undertaker, a native of this city residing at 442 Jackson
Ave. who hereby declares, that

 Charlotte Doyle Barrett (white)

a native of N. O. La. aged 66 years,
departed this life, yesterday (9 December 1925) at 929 First St in this city
Cause of Death Arterio Sclerosis. Hemorrhage of Brain.
Certificate of Dr. W. E. Graves
Deceased was the Widow of Andrew J. Barrett, and the Daughter of Geo. Doyle,
and Francis Mountain, A native of Eng.

Thus done at New Orleans, in the presence of the aforesaid D. A. Ingersoll
as also that of Messes P H Lanauze & W. ? Prudhomme both of this city
witnesses, by me requested so to be, who have hereunto set their hands,
together with me, after due reading hereof, the day, month and year first
above written.
