Be it Remembered, That on this day to=wit the Twenty eight of February in
the year of our Lord One Thousand Nine Hundred and the One Hundred and
Twenty fourth of the independence of the United States of America before me
Quitman Kohnke, M.D. President Board of Health and Ex-Officio Recorder of
Births, Deaths and Marriages, in and for the Parish and City of New Orleans,
personally appeared :

Mrs. A Leity Jr an undertaker, a native of this city residing at
2241Magazine street who hereby declares, that

 Corlis M. Barrett (white)

a native of this city aged 17 years,and 11 months
departed this life, on the twenty sixth (26 Feb 1900) at 536 Second street
in this city
Cause of Death La Grippe
Certificate of Dr. T. O. Dabney
Deceased was the lawful son of A.J. Barrett and Charlotte Doyle

Thus done at New Orleans, in the presence of the aforesaid Mrs. A Leity Jr
as also that of Messes P H Lanauze & ?????? both of this city witnesses, by
me requested so to be, who have hereunto set their hands, together with me,
after due reading hereof, the day, month and year first above written.
