Be it Remembered, That on this day to=wit the Tenth of June in the year of
our Lord One Thousand Eight Hundred and Ninty four and the One Hundred and
Eighteenth of the independence of the United States of America before me S.
R. Olliphant, M.D. President Board of Health and Ex-Officio Recorder of
Births, Deaths and Marriages, in and for the Parish and City of New Orleans,
personally appeared :

Thomas Harmon an undertaker, a native of this city residing at cor Julia and
Magazine streets who hereby declares, that

 Catherine Brady lawful wife of Charles Miltimore (white)

a native of Dublin, Ireland aged 70 years,
departed this life, yesterday (9 June 1894) at his aforesaid no 329 Tulane
Avenue in this city
Cause of Death Sinicity
Certificate of Dr. W. A. Harmon
Deceased was a resident of this city for 53 years.

Thus done at New Orleans, in the presence of the aforesaid T. Harmon as also
that of Messes P H Lanauze & ?????? both of this city witnesses, by me
requested so to be, who have hereunto set their hands, together with me,
after due reading hereof, the day, month and year first above written.
