BE IT REMEMBERED, that on this day to wit:  the Sixth of December in the year of our Lord, 
One thousand, nine hundred and ten and the one hundred and 35 of the Independence of the 
United States of America, before me, W. T. O’Reilly, M. D. Chairman of the Board of Health 
and Ex-Officio Recorder of Births, Deaths and Marriages, in and for the Parish of Orleans, 
and the City of New Orleans, personally appeared
P. J. McMahon, an undertaker, native of this city residing at ___________ who hereby declared 
that Helen Campbell (white) a native of this city aged 1 month departed this life yesterday 
(5 December 1910) at 2728 Orleans Avenue in this city
