BE IT REMEMBERED, that on this day to wit:  the Twenty-third of January in the year of 
our Lord, One thousand, eight hundred and Ninety-one and the one hundred and 15 of the 
Independence of the United States of America, before me, S. R. Olliphant, M. D. Chairman 
of the Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages, in and for 
the Parish of Orleans, and the City of New Orleans, personally appeared
H. L. Frantz, an undertaker, a native of Alsace, residing at No. 155 N. Peters Street, who 
hereby declares that Hugh Campbell (white) native of Ireland, aged 61 years departed this 
life on the twenty-first instant (21 January 1891) at No. 80 S. Franklin Street in this city
