Be it Remembered, That on this day, to-wit the Twenty Second of September in
the year of our Lord One Thousand Eight Hundred and Ninety Nine and the One
Hundred and 24 of the independence of the United States of America, before
me,

QUITMAN KOHNKE, M.D., Chairman Board of Health and Ex-Officio Recorder of
Births, Deaths and Marriages in and for the Parish, personally appeared:

A.F. Bultmann, an undertaker native of this city residing at No. 2911
Magazine who hereby declares that

Mrs. Josephine Cornu, born Ory (white)

a native of Laplace, La, aged 22 years, 6 mos., 1 day, departed this life
this day (22 Sept 1899) at No. 4623 Annunciation Street in this city.

Cause of death: Child Birth.

Certificate of Dr. J.I. Hunter[?].

Thus done at New Orleans, in the presence of the aforesaid A.F. Bultmann. as
also in that of  Messrs. P.H. Lanauze and Jos. David both of this City,
witnesses by me requested so to be, who have hereunto set their hands,
together with me, after reading hereof, the day, month and year first above
written.
