Be it Remembered, That on this day to wit: the Twentieth of March in 
the year of our LOrd, One Thousand, Nine Hundred and Six and the One 
Hundred and 3oth of the Inedpendance of the United States of America, 
before me, Quitman Kohnke, M.D., Chairman of the Board of Health and 
Ex-Officio Recorder of Births, Deaths and Marriages, in and for the 
Parish of Orleans, and the City of New Orleans, personally appeared 
P.J. Shoen an undertaker native of this city residing at 521 Elysian 
Fields Ave. who hereby declares that 

		CARLOS CARRERAS (White) 

a native of this city, aged 19months, 18days, departed this life yesterday. 
(19 March 1906) at no 2021 N. Roman Street in this city. Cause of Death 
Arsenical Poisoning. Certificate of Dr. S. A. Chartomet?. Name of parents 
G.W. Carreras, Josephine Varion.  Birth place of parents La.  Thus done at 
New Orleans, in the presence of the aforesaid P.J Shoen. as also in that of 
P.H. Lanauze and E.M. Irwin both of this city, witnesses requested so to be, 
who have hereunto set their hands together with me, after reading hereof this 
day, month and year above written.
