Choppin struck out) Jos. James M.D., President Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages, in and for the Parish of Orleans, personally appeared J.R. Ferrer, a native of Spain residing at # 157 St. Anthony St who hereby declares, that ROSALIA CARRERAS (White) a native of this city aged six months departed this life yesterday (2 June 1880) at # 147 Frenchmene Street in this city. Cause of Death, Meningitis, Certificate of Dr. J. Loell Orto. Thus done at New Orleans, in the presence of the aforesaid J.R. Ferrer as also in that of Messrs. H. Peralta and P.H. Lanuaze, both of this city, witnesses by me requested so to be, who have hereunto set their hands together with me, after reading hereof this day, month and year above written.
