Be it Remembered, That on this day to wit: the Sixteenth of June in 
the year of our Lord, One Thousand, Nine Hundred and Twenty five and 
the One Hundred and 49 of the Inedpendance of the United States of 
America, before me, (struck out JOHN CALLAN) W.H. ROBIN, M.D., Chairman 
of the Board of Health and Ex-Officio Recorder of Births, Deaths and 
Marriages, in and for the Parish of Orleans, and the City of New Orleans, 
personally appeared 

Geo Terry. an undertaker native of this city residing at 2811 Chartres St.
who hereby declares that 

		Shirley Marie Carreras (White), 

a native of N.O. La., aged 6 mos, departed this life this day. (16th June 
1925) at no 2109 Louisa St.in this city. Cause of Death Gastro Enteritis, 
& Toxemia. Certificate of Dr. L.J. Robin. Deceased was the daughter of 
John H. Carreras, and Cecelia Williamson, both natives of La. Thus done 
in the presence of the aforesaid Geo. Terry. as also in that of P.H. Lanauze 
and W. J. Prudhomme both of this city, witnesses requested so to be, who have 
hereto set their hands together with me, after reading hereof this day, month 
and year above written.
