Be it remembered that on this day to wit the Twenty first of November in the 
year of our Lord One Thousand Eight Hundred and Eighty Seven and the One 
hundred and twelfth of the Independence of the United States of America before 
me, Joseph Holt, M.D., President Board of Health and Ex-Officio Recorder of 
Births, Deaths and Marriages in and for the Parish of Orleans, personally 
appeared 

Wm. J. O'Donnell, an undertaker a native of this city residing at 321 
Poydras Street who hereby declares that 

	Mrs. Widow Sarah McClain Clark (white) 

a native of Rhode Island aged 88 years 8 months departed this life yesterday 
(20 Nov. 1887) at 362 S. Franklin Street in this city.
Cause of Death Senility
Certificate of Dr. S.M. Angell
Deceased was a resident of this city for 67 years

Thus done at New Orleans in the presence of the aforesaid W.J. O'Donnell as 
also in that of Messrs P.W. (??????) A.A. Voorhies both of this City 
witnesses, by me requested so to be, who have hereunto set their hands 
together with me, after reading hereof, this day, month and year first above 
written
