Olliphant, M.D., President Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages, in and for the Parish of Orleans, personally appeared J. Lertz, an undertaker, a native of this city, residing at 825 Tchopatoulas St.,who hereby declares that JOHN DOW (white), a native of Plymouth, England, aged 54 years departed this life yesterday (25 March 1894) at No. 19 Third Street in this city. Cause of Death: Phythisis pulmonalis Certificate of Dr. E.J. Graves Deceased was married 30 years Thus done at New Orleans, in the presence of the aforesaid J. Leitz as also in that of Messrs. S.H. Larage (?) and L.B.
