Be if Remembered, that on this day to-wit: the _______ of September  in the
year of our Lord One Thousand Nine Hundred Eighteen and the one hundred and
43 of the independence of the United States of America, before me, Wm. H.
Robin, M. D. Chairman, Board of Health and Ex-Officio Recorder of Births,
Deaths and Marriages in and for the Parish of Orleans, Personally appeared
T. N. Donnelly, an undertaker, native of this city residing at 527 Elysian
Fields, who hereby declares that HENRY R. DUCASTAING (white) a native of
NOL, aged 59 yrs., departed this lifethis day (4 Sept 1918) at No. 812
Orleans St. in this city.

Cause of Death: Cerrhosis of Liver
Certificate of Dr. H A. Gabert

Deceased was married to REBECCA CAMPBELL, a clerk and son of B. Ducastaing
& Jeanne Cazaudon, birth place of parents France.

Thus done at New Orleans in the presence of the aforesaid T. N. DONNELLY
as also in that of Messrs. P. H. Lanauze and L. S. Francke, both of this
city witnesses by me requested so to be, who have hereunto set their hands,
together with me, after reading hereof, this day, month and year first
above written.
