Donnelly, an undertaker, native of this city residing at 527 Elysian Fields, who hereby declares that HENRY R. DUCASTAING (white) a native of NOL, aged 59 yrs., departed this lifethis day (4 Sept 1918) at No. 812 Orleans St. in this city. Cause of Death: Cerrhosis of Liver Certificate of Dr. H A.
