H. Robin, M. D. Chairman, Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages in and for the Parish of Orleans, Personally appeared RAOUL J. LLOPIS, an undertaker, native of this city residing at No. 1309 St. Philip Street, who hereby declares that ADELAIDE McCARTHY, widow of FRANCOIS DUGUE (colored) a native of New Orleans, La., aged 83 yrs, 2 mo. 1 day, departed this life yesterday (27 March 1919) at NO. 1614 St. Ann Street in this city. Cause of Death: Chronic Endocarditis; bronchial asthma Certificate of Dr. E. A. Rappannier Deceased was a daughter of JERRY McCARTHY and ANNA HERNANDEZ, both natives of Virginia. Thus done at New Orleans in the presence of the aforesaid Raoul J. Llopis as also in that of Messrs. P. H. Lanauze and W. J. Prudhomme both of this city witnesses by me requested so to be, who have hereunto set their hands, together with me, after reading hereof, this day, month and year first above written.
