BE IT REMEMBERED, that on this day to-wit:  the Second of July in the year of our Lord 
One Thousand Nine Hundred and Three and the One Hundred and 27 of the Independence of 
the United States of America, before me, Quitman Kohnke, M. D., Chairman Board of Health 
and Ex-officio Recorder of Births, Deaths and Marriages, in and for the Parish of Orleans, 
personally appeared
H. Rolle, an undertaker, native of this city, residing at No. 255 S. Clairborne Ave, who
hereby declares that John Joseph Farrell (white) a native of this city, aged 43 years 
departed this life yesterday (1 July 1903) at No. 428 S. Galvez Street in this city.
Cause of Death:	Atrophic Cirrhosis from Alcohol
