Be it Remembered, That on this day to-wit: the nineteenth of November in the 
year of our Lord one thousand eight hundred and ninety eight and the One 
Hundred and 21 of the independence of the United States of America before me 
Quitman Kohnke, MD, Chairman Board of Health and Ex-Officio Recorder of 
Births, Deathe and Marriages in and for the Parish of Orleans personally 
appeared Henry Tharp an undertaker, native of this city residing at corner of 
Julia and Magazine who hereby declares, that Ellen Fitzsimmons lawful wife of 
Thomas O'Connor (white) a native of County Monahan Ireland, aged 58 years 
departed this life yesterday, (18 Nov 1898) at 518 Julia Street in this city
