Death Certificate for Gaignard, Josephine (Mrs. Francois Chaumet)

Be it remembered, that on this day, to wit:  the Twenty second of 
September in the year of our Lord one thousand eight hundred and 
sixty five ...in and for the Parish and City of Orleans, personally 
appeared 

Charles A. Sabuzan, Jr. a native of New Orleans residing at #122 
Toulouse street who by these presents declares that

Mrs. Francois Chaumet born Josephine Gaignard a native of Louisiana 
aged about fifty eight years Died on the twenty first instant 
(21st September 1865) at [six] o'clock PM At #218 Frenchmen street.  

THUS DONE at New Orleans in the presence of the aforesaid Charles A. Sabuzan, Jr. ...
