Be if Remembered, that on this day to-wit: the Eleventh of November in the
year of our Lord One Thousand Eight Hundred and Ninety-Nine and the one
hundred and 24 of the independence of the United States of America, before
me, Quitman Kohnke, M. D. Chairman, Board of Health and Ex-Officio Recorder
of Births, Deaths and Marriages in and for the Parish of Orleans,
Personally appeared CHARLES GRUNBERG, native of this city residing at No.
124 N. Roman St., who hereby declares that CHARLES GRUNBERG (white) a
native of this city, aged 3 days, departed this life yesterday (10 November
1899) at the aforesaid residence in this city.

Cause of Death: Cyanosis
Certificate of Dr. S. Gilbert

Deceased was the lawful son of deponent with Jennie Campbell, a native of
this city.

Thus done at New Orleans in the presence of the aforesaid. C. Grunberg as
also in that of Messrs. P. H. Lanauze and Jos. T. O’Dowd both of this city
witnesses by me requested so to be, who have hereunto set their hands,
together with me, after reading hereof, this day, month and year first
above written.
