Be it Remembered, that on this day to-wit: the Fifth of October in the year
of our Lord One Thousand Nine Hundred and Eight and the One Hundred and 33
of the independence of the United States of America, before me W. T.
O’Reilly, M. D., Chairman, Board of Health and Ex-Officio Recorder of
Births, Deaths and Marriages, in and for the Parish and City of New
Orleans, personally appeared

CHARLES GRÜNBERG, native of this city, residing at 765 N. Miro, who hereby
declares that JENNIE CAMPBELL, wife of CHARLES GRÜNBERG (white) a native of
this city, aged 30 years departed this life yesterday 4 October 1908 at No.
765 N. Miro St. in this city.

Cause of Death: Illegible

Birth place of parents Ireland

Thus done at New Orleans, in the presences of the aforesaid C. Grünberg as
also in that of P. H. Lanauze & Thos. J. Nides both of this City witnesses
by me requested so to be, who have hereunto set their hands, together with
me, after reading hereof, this day, month and year first above written.
