Be it remembered that on this day to-wit the Sixtenth of august in the year 
of our Lorn one thousand nine hundred and thirteen and the one hundred and 38 
of Independence of the Untied States of America before me W. T. O'Reilley M.D. 
Chairman Board of Health and Ex Office Recorder of Births, Deaths and Marriages
in and for the Parish and City of Orleans personally appeared 

			T. M. F---- and undertaker native of this city residing 
at No. **** Magazine Street who hereby declares that Mary Haggerty widow of Randy 
Healy (White) a native of Ireland aged 68 years departed this life yesterday 
(14 Aug 1914) at No. ------ in this city. Cause ------ Certificate of ----- 

Thus done at New orleans in the presence of the aforesaid T. M. F----- as also in 
that of P H Lanauze & T J ----- both of this city witnesses by me so to be who have 
hereto set their hands together with me after due reading hereof the day, month and 
year first above written.
