Be it Remembered, That on the day to-wit the Twenty second day of May in the
year of our Lord One Thousand Nine Hundred Eighteen and the One Hundred and
42 of the independence of the United States of America, before me Wm. H.
Robin, M. D., Chairman Board of Health and Ex-Officio Recorder of Births,
Deaths and Marriages in and for the Parish of Orleans, personally appeared
C. Heintze, an undertaker, native of this city residing 442 Jackson ave who
hereby declares that ELIZABETH BLUME, Widow of Theodore Herr (white) a
native of this city, aged 65 years departed this life yesterday (21 May
1918) at 1416 Cambronne st., City.

Cause of death     Cancer - Rectum

Thus done at New Orleans, in the presence of the aforesaid C. Heintze as
also in that of P. H. Lanauze and Emile Louis both of this city, witnesses
by me requested so to be, who have hereto set their hands together with me,
after reading hereof this day, month and year first above written.
