BE IT REMEMBERED, that on this date to-wit: the ninth of June in the 
year of our Lord One Thousand Eight Hundred and eighty-four and the 
One Hundred and eighth of the Independence of the United States of 
America, before me JOSEPH HOLT, M. D. President Board of Health and 
Ex-Officio Recorder of Births, Deaths and Marriages, in and for the 
Parish of Orleans, personally appeared 

James R. McCloskey, a native of Boston, Mass. Residing at 191 ½ Palmyra 
Street who hereby declares that 

		William R. McCloskey (white) 

a native of this city, aged 8 mos. 9 days, departed this life yesterday 
(8 June 1884) at his aforesaid residence in this city.

Cause of Death ????? (Illegible)

Certificate of Dr. J. J. Lyons
Deceased was the lawful child of deponent with Sarah Lally, a native of 
this city.
This done at New Orleans in the presence of the aforesaid J. R. McCloskey, 
as also in that of Messrs. B. B. Howard and P. H. Lanauze both of this city.
