Be it Remembered, That on this day to=wit  the Twenty third of August 
in the year of our Lord One Thousand Eight Hundred and Ninty Six and 
the One Hundred and Twenty first of the independence of the United 
States of America before me S. R. Olliphant, M.D. President Board of 
Health and Ex-Officio Recorder of Births, Deaths and Marriages, in 
and for the Parish and City of New Orleans, personally appeared :

Hy Tharp an undertaker, a native of this city residing at  cor Julia 
and Magazine streets who hereby declares, that 

		Charles Miltimore (white)  

a native of  Massachusetts  aged 77 years,
departed this life, on the twenty first ???  (21 Aug 1896) at his 
aforesaid no 3617 Magazine street in this city
Cause of Death Uraeme Counitsims ???
Certificate of Dr. ?? G Armstrong
Deceased was a resident of this city for 54 years , a bar keeper .

Thus done at New Orleans, in the presence of the aforesaid H. Tharp  
as also that of Messes P H Lanauze & ?????? both of this city witnesses, 
by me requested so to be, who have hereunto set their hands, together 
with me, after due reading hereof, the day, month and year first above written.
