Be it Remembered, That on this day to-wit the Twenty first day of MAy in the 
year of our Lord One Thousand Nine Hundred and Eleven and the One hundred and 
35 of the independence of the United States of America, before me W.T. 
O'Reilly, M.D. Chairman of the Board of Health and Ex-Officio recorder of 
Births, Deaths and Marriages in and for the Parish of Orleans personally 
appeared Kay Tharp(?) an undertaker native of city residing corner Washington 
and (?) who hereby declares that Thomas O'Connor (white) a native of this city 
aged 71 years 10 months 27 days departed this life yeasterday (20 May 1911) at 
No. 518 Julia Street this city 
Cause of death penetrating gun shot of (?) and head, suicide
Certificate of Dr. Joseph O'Hara(?) coroner
Deceased was (?) and Chief Engineer of Fire Department

This done at New Orlenas in the presence of the aforesaid K Tharp(?) as also 
in that of P.H. Lanauzi and T.J. Niues(?) both of this city witnesses by me 
requested so to do, who have herunto set their hands together with me after 
reading herof this day, month and year first above written  W.T. O'Reilly

Thomas Jefferson O'Connor , Sr. had children form a first marriage.  His first 
wife was Ellen Fitsimmons.  She died.
