M. Batchelor, M. D., Chairman Board of Health and ex-Officio Recorder of Births Deaths and Marriages in and for the Parish of Orleans and the city of New Orleans personally appeared Geo. Desforges, an undertaker), native of this city residing 1225 N. Rampart St.. who hereby declares that Amelia Probst (wid. Of Jacob Wolf Sr.), (white) a native of N. O., La.. aged 79 yrs. departed this life yesterday (1st of June 1940) at No. 962 Taft Place, in this city. Cause of Death Chronic Cardio Valvular Disease, General Arterio Sclerosis Certificate of Dr. L. J. Robin Deceased was a widow and the daughter of Gustave Probst and Catherine Hermann, both natives of Germany. Thus done at New Orleans in the presence of the aforesaid Geo. Desforges as also in that of P. H. Lanauze & W. J. Prudhomme both of this city, witnesses by me requested so to be, who have hereunto set their hands together with me, after due reading hereof the day, month and year first above written.
