12 June 1874  William Smith, a native of New Orleans 
residing at No. 99 St. Charles St. in this city, who 
hereby declares that 

			Philip Rice, 

a native of Cincinnati, Ohio, aged 48 years and 6 
months, died the 11th instant, June 11, 1874 at 10 
o'clock a.m. at the corner of Camp and Delachaise 
Sts. In this city.

Cause of death:  Locomotor ataxy.

This done in the presence of William Smith, Jas. A. 
Davis, and H. Cohn.
