BE IT REMEMBERED, that on this day to wit:  the Sixteenth of July in 
the year of our Lord, One thousand, eight hundred and Eighty three, 
and the one hundred and 8  of the Independence of the United States 
of America, before me, Joseph Jones, M. D. Chairman of the Board of 
Health and Ex-Officio Recorder of Births, Deaths and Marriages, in 
and for the Parish of Orleans, and the City of New Orleans, 

personally appeared
C. L. Schopp, an undertaker, a native of La. residing at No. 140 La. 
Avenue who hereby declares that
