T. O'Reilly, M.D. Chairman Board of Health and Ex- Officio Recorder of Births, Deaths, and marriages, in and for the Parish of Orleans, personally appeared E.J. Mothe (?), an Undertaker, native of CITY, residing on 222 (?) who hereby declares that Isaphine Burkett, (white) Widow of John Smith a native of (?), aged 70 years departed this life yesterday, 11 Jan 1914) at no. 1011 Jackson, Algiers . She had two children by him: Elizabeth Henrietta Aikman McLean and Robert L. Aikman. She had 4 children with John Smith: John Smith, William B. Smith, Frank C. Smith and Rachel Ellen Smith Clasen.
