T. O'REILLY, M. D., Chairman Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages in and for the Parish of Orleans, personallly appeared: C. Preau, an undertaker, native of this city, residing at No. 1112 N. Rampart Street who hereby declares that Annie Stayert (White) wife of Henry Stechmann a native of New Orleans, LA, aged 32 y 6 m & 14 days, departed this life yesterday (16 February 1915) at No.2445 Burgandy St. in this city. Cause of death: Chronic Par Nephritis Certificate of Dr. E. Pollock Birthplace of parents: Father Indiana; Mother LA. This done at New Orleans, in the presence of the aforesaid C.Preau as also in that of P. H. Lanauze and T. J.
