M. Batchelor, M. D., Chairman Board of Health and ex-Officio Recorder of Births Deaths and Marriages in and for the Parish of Orleans and the city of New Orleans personally appeared W. H. Matthews III an undertaker, native of this city residing 1225 N. Rampart St.. who hereby declares that Lillian Schick (wid. Of Rene J. Legier), (white) a native of N. O., La.. aged 53 years departed this life this day (17th of June 1940) at Touro Infirmary. In this city. Resided at No. 4501 Camp St. Cause of Death Acute Pancreatitis. Pancreatic Stones.
