Be it remembered that on this day to wit, the Eighth of April 
in the year of our Lord one thousand nine hundred and Twenty-
two and the one hundred and 46 of the Independence of the 
United States of America, before me, John Callan, M. D., 
Chairman Board of Health and Ex-Officio Recorder of Births 
Deaths and Marriages in and for the Parish of Orleans and 
the city of New Orleans personally appeared

Geo. Ader (?), an undertaker, native of this city residing 
1112 N. Rampart St.. who hereby declares that 

Miss Hilda Pauline Wolf, (white) 

a native of N. O., La.. aged 27 yrs., 3 mos. departed this 
life on the seventh (?) (7th of April 1922) at Covington.

Cause of Death  Pul. Tuberculosis
Certificate of Dr. L. L. Young
Deceased was a daughter of Jacob J. Wolf and Amelia Probst, 
both natives of La.

Thus done at New Orleans in the presence of the aforesaid Geo. 
Ader as also in that of P. H. Lanauze & W. J. Prudhomme both of 
this city, witnesses by me requested so to be, who have hereunto 
set their hands together with me, after due reading hereof the 
day, month and year first above written.
