Be it remembered that on this day to wit, the Twenty-fifth of April 
in the year of our Lord one thousand nine hundred and Sixteen and 
the one hundred and 40 of the Independence of the United States of 
America, before me, W. T. O’Reilly, M. D., Chairman Board of Health 
and ex-Officio Recorder of Births Deaths and Marriages in and for 
the Parish of Orleans personally appeared

J. J. Rauch an undertaker, native of this city residing at No. 1425 
Tulane Avenue.. who hereby declares that 

		Frederick Peter Wolfe, (white) 

a native of Franklin, La.. aged 57 yrs. departed this life yesterday 
(24 April 1916) at No. 2350 Magazine street, in this city.

Cause of Death Acute Nephritis Aedema of lungs, delalation(?) of Heart
Certificate of Dr. W. A. Gellaspie
Deceased was married, a dentist Birthplace of parents, Germany

Thus done at New Orleans in the presence of the aforesaid J. J. Rauch 
as also in that of P. H. Lanauze & (?) both of this city, witnesses 
by me requested so to be, who have hereunto set their hands together 
with me, after due reading hereof the day, month and year first above 
written.
