To the Honorable Judge of the 25th Judicial District Court of Louisiana for the Parish of St. Helena: The petition of Mistress Mattie Webb, a resident of your said Parish and State, widow of Thomas Story, deceased, respectfully represents: That many years ago she was married to Tom Story, and resided with him as such until November 26th 1909, on which date the said Tom Story departed this life, at his place of residence in your said Parish and State, leaving no will or testament. That of the said marriage four children were born, as follows: Willie Emmet Story, age nine years; Otis Plaise Story, age seven years; Vera Wilmoth Story, age four years, and Andry March Story, age two years. That said deceased is survived by the above named minor children and your petitioner, his widow. Shows that as the mother and surviving parent of said minors, your petitioner is entitled to be recognized and confirmed as their natural tutrix, and to that end wishes to have an inventory of all property left by said deceased made and recorded in the mortage records; and to have an undertutor appointed for said minors, all as provided by law. Shows that petitioner knows of no property owned by the said deceased at the time of his death, but that your petitioner desires that a commission issue to John B. Dean, Clerk of Court and ex-officio Notary Public for the Parish of St. Helena, Louisiana, authorizing him to make an inventory in case he shall be able to find any property belonging to said deceased. Avers that G.E. Story, a resident of your said Parish, is a suitable person to be named as under-tutor for said minors, he being petitioners' grandfather, and great grandfather of said minors. Wherefore petitioner prays that the Court order an inventory made of the property belonging to said deceased, for its recordation as required by law, and in due time, for the appointment of petitioner as natural tutrix for said minors and of G.E. Story, as undertutor for said minors, and that upon such recordation and the taking of their respective oaths of office, letters as such issue to petitioner and said Story, as provided by law. Prays for all necessary orders and general relief, By,(Signed) Krup & ?, Attorneys for petitioner ORDER: The above petition considered, it is ordered that a commission issue to John B. Dean, Clerk of Court and ex-officio Notary Public for the Parish of St. Helena, Louisiana, authorizing and commanding him to make an inventory of any property found by him belonging to the deceased Tom Story, to be assisted therein by two appraisers by him appointed, and how he shall have executed this commission, he make due return: It is ordered that upon his recordation of his return on said commission in the mortage records of St. Helena Parish and upon the petitioner Mistress Mattie Webb Story and G.E. Story, taking the oath required by law as tutor and under-tutor for the minors named in the foregoing petition, that letters as such do issue to them: All in the manner required by law and as prayed for. (Signed) Robt. J. Ellis, Judge of said Court Amite, Louisiana, August 31st 1910
