Jerry Lee Hebert, age 56, passed away on Thursday, July 28, 2011. 

Beloved father of Jonathon R. Hebert and Jeremy G. Hebert. Brother of Carolyn 
LeBlanc, Joyce Hebert, Paulette Clavier and Floyd Hebert, Jr. Also survived a 
host of nieces and nephews. 

A native of New Orleans, LA and a resident of Slidell, LA for 30 years.

Relatives and friends are invited to attend the Memorial Service on Saturday, 
August 6, 2011 at 10:30 AM at High Praise Fellowship, 60456 N. Military Road, 
Slidell, LA. Visitation on Saturday at the church from 10:00 AM until service 
time.

In lieu of flowers, donations may be made to Children's Hospital New Orleans
200 Henry Clay Avenue New Orleans, LA 70118 in Jerry's Memory
