Jeremy Michael Hebert went home to be with Jesus on Thursday, September 29, 
2011, after battling heart disease for many years.

Beloved son of Marius Joseph Hebert and Ann Eileen Bates Hebert; and loving 
brother of Daniel Hebert and Melissa Swartzfager. He will also be remembered by 
many family and friends. He was preceded in death by his grandmother, Ruby 
DupreÕ Hebert.

Jeremey was a 12 Year old Sixth Grade student at Pineview Middle School in 
Covington, LA.

Relatives and friends of the family are invited to attend a Celebration of 
JeremyÕs Life on Sunday, October 2, 2011 from 6:00 to 8:00 PM at Trinity 
Evangelicial Church, 19380 North 10th Street, Covington, LA 70433.

Graveside Services will be held at Pinecrest Memorial Gardens on Monday, 
October 3, 2011 at 10:00 AM.

In lieu of flowers, the family would prefer donations to the Jeremy Michael 
Hebert Trust Fund, at Whitney Bank. The trust fund has been established for the 
specific purpose of assisting with the placement of Heart Defibrillators at all 
St. Tammany Parish Schools.
